Director of Wellness - Generations at Ahwatukee

Duties and Responsibilities:

  • Manage the day-to-day healthcare operations of this IL/ALZ Community to ensure residents’ healthcare needs are met
  • Provide overall clinical leadership to care services team
  • Coordinate nursing/resident services to ensure the continuity of the resident’s total regimen of care
  • Recruit, hire and train team members to ensure the highest quality of care is consistently provided
  • Supervise all care staff, including interviewing, evaluating, training, mentoring, assigning work schedules, developing performance improvement plans
  • Participate in the development, implementation and enforcement of policies, procedures, and systems in accordance with state and federal guidelines and company standards
  • Execute the Quality Assurance Program
  • Adhere to the department’s budgetary obligations
  • Effectively Communicate with physicians and family members
  • Resolve concerns regarding resident care with all constituents in a timely manner
  • Provide hands-on resident care as needed or warranted
  • Perform ongoing nursing assessments/observations of residents’ physical and psycho-social needs, and coordinate with other departments to assure quality care
  • Provide continuity of care for those residents receiving home health care, hospice services, and other third party healthcare-related services
  • Ensure proper and timely records reporting in your department in accordance with state and federal guidelines and company standards
  • Supervise and coordinate admissions, assessments, and care plans to assure accuracy, timeliness and completeness so they are personalized
  • Perform on-site assessments of residents admitted to hospitals or skilled nursing facilities for treatment, and maintain contact with resident’s family and discharge planner with the view to returning to the community
  • Ensure Resident Confidentiality and HIPPA regulations are compliant
  • Document and report care provided and resident observations including unusual or significant changes in physical, mental or behavioral conditions
  • Maintain a working knowledge of state regulations, policies and procedures, dictated for compliance
  • Assist with medication and treatment administration and ensure medications are administered per physician orders As directed, review and coordinate completion of incident reports that are health and injury related and develop/monitor plans of correction in accordance with policies and procedures

Qualifications

  • Associate’s or Bachelor’s degree strongly preferred
  • Active RN or LPN licensure in the State of Arizona. Must be in good standing
  • Minimum of three years of progressive Nurse Management experience in Assisted Living including experience working with dementia or Alzheimer’s residents
  • Demonstrated working knowledge and experience with Geriatric Nursing and care services for a wide variety of residents
  • Strong interpersonal skills with the ability to communicate effectively to a wide range of audiences, both internal (care staff, nurses, executives, etc.) and external (families, residents, physicians, vendors, etc.)
  • Must be able to work occasional nights/weekends and holidays as required and to be on call via telephone for nursing related emergencies
  • Current Fingerprint Card
  • Current First Aid and CPR certifications
